I mean "tell no one" means pretty clearly "tell no one." You might choose to tell someone for worthy reasons, but you're obviously violating the request, and if you respond "of course" to the request, then you have dishonesty betrayed the person's confidence.
Not sure if it's the neurodivergence, or being queer and some sort of poly/not invested in 'spouse being Other Half' as a concept, but reading all the B's is so viscerally upsetting lol.

Like, it's a major breach of ethics for therapists to tell their spouses what their patients said to them.
That's what confidentiality means.

A- You’re not telling your spouse/partner
B- You are telling your spouse/partner unless you’ve been specifically asked not to
