The horrors he dreams of are often laced with something comically dramatic. Not really dreaming about ripping someone open, but dreaming about polymorphing Astarion into a sheep, unable ever to reverse it. Dreaming that he's forced into slavery in Menzoberranzan as a mushroom trader...
Of course, there are also the urges + Bhaal's influence, but these silly yet scary dreams are unrelated, appearing even after he rejects his heritage.
They still wake him up sweating and checking his surroundings; as goofy as they seem once he wakes up, they feel frighteningly real in the dream.
