Solid tumors develop dense stromal barriers that limit drug delivery and immune attack. Qu et al. engineer MDK-targeted EV carrying a photosensitizer that, upon light activation, simultaneously reprograms the CAFs and immune microenvironment. This dual stromal-immune remodeling markedly enhances the efficacy of immunotherapy and chemotherapy in preclinical models.
