Alright so it looks like bsky pushed an update (I did have to update the app) because the number of properly tagged posts suddenly exploded. I'm going to stop it here then because it also looks like bsky stopped cracking down on people as hard as well. They "fixed" it, it seems. (1/4)
It also looks like it now properly shows warnings despite whatever moderator settings you have on, which is much better IMO. Biggest sign something has changed is the What's Hot page. No idea wtf changed but, something 100% has changed. Here's the Following, Discover, and What's Hot results. (2/4)

I used the same settings through the test (all tags set to "warn") and never adjusted them, so it very much seems like there was either a bug making things not display right, or they started being much more aggressive to everyone in general about it. Regardless, over 50% is much better! (3/4)
