Wondering how other institutions have handled transition to IDMS SCr in terms of abx dosing for patients studied prior to IDMS SCr? Historically I’ve used a calculator to adjust CrCl to non-IDMS based value (lowering CrCl ~10-15%) but wondering how necessary this is clinically. Thoughts? #IDsky
