In Neno, Malawi, the current fuel crisis is affecting nearly every part of the health system. Ambulances have been delayed. Surgeries have been pushed back. Home visits for palliative care patients have been disrupted.
A fuel shortage is not abstract when a newborn is waiting for transport.
We talk a lot about what health systems need: doctors, nurses, medicines, hospital beds, equipment.
But sometimes the thing standing between a patient and care is much more basic.

When a maternal and child health outreach clinic is cancelled, people do not just miss an appointment. They may miss antenatal care, childhood immunizations, HIV testing, or treatment for something that will get worse if left untreated.
The cost of disruption is carried by patients.
